fortls not initialized when using fortran extension

You list two options as requirements: Modern Fortran or fortran extension.

However, when using the fortran extension (by Xavier Hahn), fortls doesn't seem to get initialized for .f90 files.

Tested on versions: vscode 1.34.0 FORTRAN IntelliSense 0.5.0 fortran 0.2.0
