add automatic gain control #218

Similar to the (current/future) implementation of auto-MTF adjustment, the decoder should adjust sync and white levels to follow what's actually on the disk. The former will help with #217, and the latter will ensure proper brightness in the final decodes.

Fixing this should take care of the Blackadder issues raised in #252

Added a sample from the Swedish Karaoke disk that dramatically slows down and fails to decode.
