SALVIA:
  Put window handle into renderer_parameters.
  Now texture hold surface pointers but not vector of surface. (STILL RUNNING ERROR)
  Remove unused hdev for renderer creating.
  Update performance document.

  New swap chain works on PartOfSponza.
  Remove D3D9 and GDI+ support.
  Remove salviax_swap_chain project.
  Swap chain was merged into salviax as static lib.
  OTHER SAMPLES CANNOT BE COMPILED!

  Rename brand_renderer to render_core.
  Add command id to render state.
  Remove primitive count parameter from vertex_cache::transform_vertex and rasterizer::draw.
  Fetch from index buffer now enabled or disabled by command id but not index buffer is null.
