I installed Diluvian and noticed that no teleporter spawns when playing in this difficulty. I traced the issue to Classic Items - when this mod is disabled, teleporters spawn, and when only Diluvian and Classic Items are enabled, they do not spawn.
This was in single player. Here is my output log for one of the testing sessions with ClassicItems and Diluvian enabled:
