Upgrade to Rails 4 #7

Upgrade to Rails 4.1.4 and ruby 2.1.2. Keeping in rails_4 branch per #5

Also added the annotate gem which just makes life easier in general. :smile:

This looks great. I just want to test this myself on Heroku and ill merge this in sometime this week. Thanks for this!

Sounds great - I'm in no rush, it's already in use for our purposes, merge/report bugs/fix issues at your leisure. :)
