"+" (plus sign) in filename, Code Review Link is dead in Issues. #27

This plugin has the same problem as the following URL: https://www.redmine.org/issues/37718

It seems to incorrectly convert "+" to "%20" instead of "%2B" when generating URLs for code review links.
