To better align with the platform design, this moves the book title and other metadata outside the white content area:

Addresses #1877; left nav section anchors now appear at the same vertical alignment as in the content flow. Previously they appeared too high up.
Reworks the TOC layout to be more resilient to varying length chapter titles by being less proscriptive.
Previously TOC links to top-level chapters would not jump you down to the start of the content; now all TOC links go directly to the content start.
