One of the suggested improvements for reading mode from our student testers was to make it easier to move between sections while deep inside a given chapter. The reading mode TOC only appears at the top (because it was designed with linear reading in mind), but the left nav real estate is available and could be put to use here.
As a first stab:
Use the left nav space to provide a fixed TOC view.
The current section should be highlighted in some way.
Remove the "back to TOC" floating button.
Experiment with how deep of a TOC to show and how much content should be shown per line.
It may be nice to provide an expand/contract feature for chapters outside of the current one, or that could be UI overkill.
