Add Github contribution guidelines file #343

Created a CONTRIBUTING file as per the Github pattern so that the guidelines appear automatically for those creating a pull request through Github.

The format and content was taken from the Vlad documentation project with formatting changes and additions based on good practice.

Fixes #342

I didn't even know that there was a GitHub pattern - pretty obvious now that I actually stop to think.
