This can happen due to a weird issue with the toolkit vs driver version, and is often present in the drivers downloaded/installed from the CUDA apt repo around certain updates. I would suggest uninstalling/purging the drivers and toolkit and installing the latest from here: https://developer.nvidia.com/cuda-downloads as well as generally avoiding the apt repo or other packaged driver installations for this. I'm not sure why the packaged versions have issues but it's somewhat common.
