Patch HCP Consul/HCP Vault tutorial

There were some blockers that prevent successful deployment:

• Unique names were not enforced
• kubeconfig generation wasn't consistent
• kube cluster context wasn't set right

Added:

• programmatic generation of consul-values
• makefile for easier deployment
