This PR fixes #209 and partially addresses #216. Specifically, it:

adds a reference for Druzdel 2003 to provide evidence and further reading for the chapter's claims about what to do in the face of subgroup heterogeneity in one's causal graph,
separates the utility equations in section 2 so they can all be labeled and still allow the PDF to compile
push the updated PDF to the repo,
fix a small typo by putting a comma inside quotation marks.

After I merge this PR, I'll make a release and send the update to Stephane (and whoever is working with him as co-editors).
