Add missing index pages #194

For every directory level in our current directory structure, there should be an index page of some kind, rather than a 404. Google's crawling information gives a hint about which are needed:

I started writing an archive view for /ann/nomcom/ but realized that there was one already - http://www.ietf.org/nomcom/index.html so what do you think about just adding a redirect?
