What was the last version this was functional on? Are you able to run network traces or provide logs?

I am having this issue as well, although my situation is a bit different. I had a stand alone controller on a raspberry pi that died so I set up the HA add on. I can get to the dashboard and have managed to get the checkbox for the override inform address set. however after the USG-3p is found and provisioned it goes to "Disconnected" going to the USG-3g internal config page shows a red banner at the top that says the "controller is currently unreachable". I have tried factory reset of the USG-3p and readopt but I just get the same issue. Is there a way to make sure that the HA-OS has all of the ports needed open ( https://help.ui.com/hc/en-us/articles/218506997-Required-Ports-Reference )

EDIT: did another factory reset on the USG-3p then went 192.168.1.1 and manually updated the inform URL to

http://_ip of HA_:8080/inform

it then connected and adopted however none of my switches or APs adopt now even after factory reset and I don't know how to get to their config pages.

EDIT 2: Workaround found - for what ever reason after doing a factory reset on each device and attempting adoption I found that after the device failed adoption if I did "provision" (which should not work until adoption succeeds) it would then connect and provision
