Additional options for feedback forms

Added two new form components and renamed the original form:

• FeedbackFormBasic - the original
• FeedbackFormCatalog - Catalog Quality Corrections
• FeedbackFormContent - Content Quality Corrections

These new forms follow guidelines/specs from Val's requirements doc.
