In-call volume doesn't appear to work

What steps will reproduce the problem?
1. Make a call
2. Use volume rocker

What is the expected output? What do you see instead?
Volume should increase or decrease with volume rocker. It does not.

What build are you using? Do you have apps-on-sd? What radio? Did you wipe
after flashing (yes if you are on an auto-wipe build)?
Wiped after flashing. No apps-on-sd. 5.0.2H

Please provide any additional information below.
