BOOKING PAGE: Customized form for each selected service.

Form information: Name Email Phone number Organization name (optional) Select service (dropdown): career consulting, digital marketing, decision making, public speaking If they select career consulting, it will prompt them to pay them to calendly link where they can choose and pay directly. If they choose public speaking, it should prompt them to answer another question ‘Enter event information including event name and type (virtual/in person), date and location. If they choose decision making or digital marketing, it should prompt them to provide more details about the project.

Upon completion of the form - they will get an automated thank you email.
