Innotation control should be hidden for pdfs #143

The control for specifying the placement of innotations---margin, footnote, etc.---appears even when annotating converted pdf documents, where those positionings don't work. The control should not be shown in that case.

We should have a field in the model for whether a given document is html or pdf, and adjust accordingly.

@karger Can you please provide a way to reproduce that behaviour. What document are you working with and I'll look into that.
