Simplify Dev Workflow description and remove QA branches #27

Rewrite description of the workflow to be more concise
- List out the steps in a concise numbered list, rather than length paragraphs
  - Remove code blocks that can be reasonably assumed
- Leverage succinct footnotes rather than full sections to explore topics like discretionary releases
Remove section that references QA branches
- We are no longer mandating separate QA branches
- Instead we just leverage the existing release branch

@KrishnaKulkarni w.r.t. our move to better documentation, I am going to take your changes here, and add them to the healthify/healthify wiki. I will import your commits, then make a commit with my suggestions from this PR.
