Internal Server Error when no info filled in

Steps:

• Log in as clinician
• Click on teacher from caseload
• Click on Details & Meds
• If no school feedback has been filled in yet, you get an internal server error.

If you:

• Login as teacher
• Fill in some data on the school feedback form & save
• Log back in as clinician and repeat steps above - it then works fine.

Could you please provide the login details you used to test this?
