Remove all texts related to US Core #6

Since we are using US Core generator it generates some US Core-specific narratives like this: image(3) ONC is the USA specification and it should not be present in the text.

Another example is USCDI. "In order to support USCDI, servers must support all USCDI resources." in 2.1.05.

Also please check for other US Core-related texts.

We should use tx.test.hl7.org.au instead of tx.fhir.org
