Awesome code, just experiencing one issue if you can help me with it

So I'm using a form that has multiple radio buttons options. I am successfully recording the radio values in Google Docs with the help of your awesome code. The only problem is that the last question is an "other" choice with a text input and I dont know how to record the answer to the "other" option. Here is the form code:

Your best bet is to create another field for "Other"
