Burn Optical Disc: :-( unable to WRITE@LBA=...: Input/output error #103

At the end of the writing process we sometimes get:

:-( unable to WRITE@LBA=...: Input/output error

Would a full blanking of the disc solve this? If yes, we need to add this feature to Burn Optical Disc.app.

Also stops in the middle of blanking the disc when using dvd+rw-format on the command line:

FreeBSD% dvd+rw-format -force=full -blank=full /dev/cd0 
* BD/DVD±RW/-RAM format utility by <appro@fy.chalmers.se>, version 7.1.
* 4.7GB DVD-RW media in Sequential mode detected.
* blanking 9.4%:-( unable to TEST UNIT READY: Input/output error

Defective disc and/or writer?

Same disc, totally different writer:

Users-RevoOne-RL85% sudo dvd+rw-format -force=full -blank=full /dev/cd0 
* BD/DVD±RW/-RAM format utility by <appro@fy.chalmers.se>, version 7.1.
* 4.7GB DVD-RW media in Sequential mode detected.
* blanking 0.0-:-( unable to PERFORM OPC: Input/output error
-:-( unable to BLANK: Input/output error
