This PR updates the README.md according to the guideline set forth here.

To see the new README.md without the diff: Readable.

I've left the Ruby specific bits (like the application contract and known differences) unchanged. So this PR is mostly applying the same header and footer content as other Heroku CNBs (like https://github.com/heroku/buildpacks-nodejs).
