Include the URL to two-factor documentation in error messages #36

when I need 2fa on a machine please include a link to somewhere i can get that in the response message

so instead of: ! Two-factor required for this session.

do: ! Two-factor required for this session. (See: http://devcenter.heroku.com/blahblahblah)

Fixed by heroku/api#2167. We'll probably be reverting this pretty soon though as 2FA comes into Toolbelt.
