improve setup instructions

This project is awesome and I got it working after spending a lot of time on it.

I am supplying this pull request to improve the instructions on the parts I got tripped up on:

• I selected the wrong account type in the Microsoft Azure configuration, so I made that clearer with a screenshot.
• I used the wrong redirect URI. I followed the instructions and left the index.php in the url. But when I did that I couldn't register the app, the redirect kept failing.
