Ability to create hMix by age ranges + height + gender for health metrics #18

Healthy ranges per metric may vary by a combination of age range + height + gender.

We need to allow physicians to be able to create healthy ranges for specific demographics and have an easy way of viewing + manipulating healthy ranges at that scale.
