TODO:

[x] handle LSP errors gracefully, i.e. if the LSP server fails, log the error and continue operating, e.g. return file contents without diagnostics
[ ] add documentation for LSP
[ ] add tests

Docs and tests will be addressed in another PRs. I want to make a big overhaul of both.
