Changing the resolution or display mode crashes the renderer in CS2 #118

When the overlay is injected and I change the resolution/display mode in CS2, the game is stuck on the last frame rendered.

did not get the resizebufer call with the correct swapchain, so they are actually create a new swapchian when resize..
