Correct numbering so it matches IAMA document #2

1.3 in IAMA has 2 parts: 1.3.1 and 1.3.2 - these are labelled as 1.3 and 1.4 in IAMA Checker
1.5 in IAMA has 3 parts - these are labelled as 1.6, 1.7 and 1.8 in IAMA Checker
Move the listing of human rights to question 4.1 with the clusters
Move all other questions up 1 (4.2 - 4.7 instead of 4.1 to 4.6)

Requires updating the database to allow float input for question number instead of integers (to allow phase 1 question 3.1) OR adding a new field called subpart. This latter might require some more editing to ensure questions are displayed properly.

Question history in phase 4 displays as "vraag 5.". This needs to be fixed to say 4.
