There is an "in game reminder to eat timer" you may be able to lift a bunch of code from: https://foundryvtt.com/packages/burger-time
This stopped being supported over two years ago. Is this feature no longer of interest?

In my opinion, there are already very useful "in game-world clock" modules—like Sample Calendar and Small Time—that can pause and manage multiple world calendars, along with different modules and quests. Continuing to write further would only duplicate existing functionalities.
At the same time, since combat in D&D is only 6 seconds per round, three hours of real-time combat may translate to just one minute of in-game time, making tracking combat time potentially less useful.
Of course, if it records how many long rests the players have taken and how much real time has been spent on combat, I think that aligns better with the direction of my "real-time" tracking module.
I would LOVE a countdown timer that is not "real world" time, but "in game time".
This would be extremely useful for things like "you roll 1d8 for every hour you are in the dungeon, if you roll a 1, a random encounter happens". Or "it's been a day hours, you need to eat".
