BeExtCodeableReference has a wrong context #137

https://github.com/hl7-be/core-clinical/issues/86 tries to use this extension on a Diagnostic resource, and this gives an error in QA. The value used currently is clearly an error, as it would assume that a BeExtCodeableReference would only be useable inside an Extension.
