Changes to FibriCheck monitoring profile

• Remove some unnecessary components (chadvasc, measurementAttempts, rmssd)
• Remove device reference for the sake of simplicity
• Introduce heart-rate value (hrValue)
• Introduce hrReport component to store measurement report URL
• Introduce identifier

Hi @costateixeira, I'm working on the Fibricheck POC. Is this the appropriate way to suggest changes to the HR monitoring profile?
