ask-at-order entries may be provided by the requester in order and if lab report is a legally signed document how would laboratory sign off smth provided by someone outside organization?
Ask-at-order information usually serves as basis for providing calculated results and selection of standard ranges for result interpretation. They basically create a clinical context for laboratory report. As such, I believe, should be part of the report. It is quite common, that health services rely on information provided by third parties. In such case, laboratory can indicate source (authorship) of the information using a standard observation attributes.
I propose to add a section with supporting information into the lab report. Supporting information should cover mainly ask-at-order entries. These should be part of the lab report as recipient of the report might not have access to the order and document should be self-containing. Such section has been added into Xt-EHR lab logical model already.
