au ballot fixes brand name extension and generic name extension #696

This PR removes the extensions Brand name and Generic name from 4 medication profiles (Medication, MedicationDispense, MedicationRequest and MedicationStatement) -refer to https://jira.hl7australia.com/browse/FHIRIG-188, removes the extensions from existing examples (refer to https://jira.hl7australia.com/browse/FHIRIG-185) and leaves the extensions only available from the extensions profile page.
