Template changes updates #752

Changes to accommodate the new template (ig.ini) build, including:

changes to text content in the intro.md's;
alignment of text in the descriptions of the profiles and extensions; .
removal of element from all structureDefinitions;
standardisation of and elements in ig.xml;
removal of examples and content that should be in AU Core;
fixes to minor typos
