Ballot: editorial fixes #754

  1. Fixes FHIRIG-197, FHIRIG-198, FHIRIG-199, FHIRIG-201, FHIRIG-202, FHIRIG-203.
  2. Removes erroneous files incl empty notes files, empty intro files, informative files for content not held in AU Base.
  3. Definitional material moves from intro files to StructureDefinition.decription.
  4. Update to examples generation script removes duplication of resource type text.
  5. Makes other typographical fixes in pages.
  6. Restructures Home page to form part of new look template with 'How to read' added, and heading restructuring.
  7. Adds missing content to ignoreWarnings.txt.
