Changes look good.

Now that the guidance has been represented as additional bindings in the profile, can we simplify the guidance to point to the bindings in the profile, for example: "When sending a discipline-specific laboratory result, one of the discipline-specific value sets added in this profile as additional bindings to Observation.code may be more suitable for use than the entire reporting value set bound to Observation.code."

That would be a different change tracker, this is only on adding additional bindings according to the JIRA.
