Moved sex and gender identity guidance from Patient, Practitioner and RelatedPerson profiles to Guidance page. Added link from profiles to guidance.

If space allows, I might suggest that the title of the guidance page, "Individual sex, gender identity and related concepts" or "Sex, gender identity and related concepts".
