Add Location resources - generate from Organization #10

Add Location sample data.

  1. Identify Locations to be generated using Organization, and Practitioner/PractitionerRole samples provided by Services Australia
  2. Assess initial gaps, add any additional Locations for 'minimum' set
  3. Design and develop those Location instances (assume csv file)

This has been done and references added to PractitionerRoles where an Organization is linked.
