Add independent resources for tests.

I decided to create an absolutely independent set of FHIR resources.

There are a few pros to doing this:

1. Now, resources only have references from this dataset (no resources already created on the server).
2. It will be easier to maintain.
3. It should reduce the time for tests because now we don't need a huge list of patient IDs. It will be enough to use just the patient with patient-tc ID

I checked all of these resources in the Inferno Validator.

Please check them and add them to the server.
