FHIR -> CDA: Composition.type -> hl7:code: missing LOINC code #32

For example for PADV, but also concerns the other eMedication exchange formats.

FHIR PADV: Composition.type requires only the SCT code, see spec

CDA PADV: But in the element hl7:code both the SCT and the LOINC code are required, see spec
