Home (Foundation): Link the documents Lab-Order Document (ServiceRequet only) and Lab-Order Document by form.

Use Cases: Split into sub-chapters Lab Order by form and Lab Order ServiceRequest and add a document example for each use case

Case Study: Everything is currently in the example. The introductory text should be excluded. The new introduces pages (one for SR only, one for Form) should have a text similar to the use cases and document and the document should be linked (XML/JSON) at the end of the correspondig text.

Artifacts: There should be an index at the front of the description text that indicates whether this example can be used for the ServiceRequest and/ or By Form.
