Error messages: LinkId 'xx.xx.xx' not found in questionnaire

During validation, there are error messages (see QA) for added items in the Questionnaire.

Eruation:

• The items are generated in the questionnaire (spelling checked).
• When removing the items from the QR, the errors disappear.
• The items in the QR are not in the positions listed in the error.

The validator checks the published version (http://fhir.ch/ig/ch-orf/Questionnaire/order-referral-form) of the questionnaire and not the version in the ci-build.
