Add special needs conditions at exit #13

DCA’s Data Quality report used for monitoring examines special needs conditions at both entry and exit; this app only looks at the special needs conditions at entry. It would be good to have the special needs conditions at exit added to this app so that agencies can clean data per DCA specifications.

Actually, DCA uses our Data Elements app to evaluate data quality now. Do we still want to add special needs at exit to the Elements app, or would that clutter things up too much? If no one objects, I'll go ahead and add them.
