[Enhancement]: Add Placeholder Text in Form Field Areas In Settings Page.

Description

There is an issue with a form field that lacks any placeholder text or labels, leaving me unsure about what information to input.

Acceptance Criteria

Add descriptive placeholder text or a label to the currently empty form field to indicate what information the user should enter.
The placeholder text, labels field should match the overall design and style of the form, maintaining consistency with the rest of the application.
