add reporter of a issue #14

Please add a field for the reporter of a review issue to the first line of the review comment, and in turn a column for the reporter in the html report.

For example:

/*TODO:review:new:severity:minor:reporter:RocketMan82
 * ...
 */

After showing the review minder to a broader audience it seems that this is one of the features we should provide soon.
