Enhancement for "new issue" template #57

Suggested enhancement to the template for reporting new issues:

1. Add an extra blank line after "*** What steps will reproduce the problem?"

makes it easier to read

2. Separate the following questions (easier for user/reader). Ensure blank
space between' em

*** What is the expected output? 

*** What do you see instead?
