Add issue templates #934

Description

This PR adds two templates that can be used when creating new issues on this repo.

Hopefully this will aid in development by increasing understanding and providing us more context around our work.

Please delete options that are not relevant.

How Has This Been Tested?

This was performed via Github web UI, I expect no failures.
