ok, the reason we are seeing this is because the modification causes a CreateTaskCommand and an UpdateAttributeTaskCommand with two different execution references (and an AddCandidateGroupsCommand, but that's irrelevant).

Further findings: When you modify an instance through the cockpit (drag & drop token to another activity), it usually puts the "start before" before the "cancel" part. This means that for a very short time, there is an extra execution and the task is created in this execution. Then the task is somehow "moved" to the original process instance execution. Somewhere after that, an extra event is fired and gets the source reference of the process instance.

Possible workaround: Swap the order of the process modifications: Cancel the old activity first, then start before the new activity. Then there will be only one source reference and the issue won't occur.
